Abstrak

Dengue Hemorrhagic Fever (DHF), transmitted by Aedes aegypti and Aedes albopictus mosquitoes, remains a pressing public health issue in Indonesia, including Ende Regency, with rising cases and fatalities. To combat this, the One House One Larvae Observer Movement (G1R1J) was introduced in 2015, promoting the 3M Plus strategy to empower households. However, its implementation faces persistent challenges due to limited public awareness and inadequate resources. This study investigates key determinants affecting G1R1J’s policy execution at the Kota Ende Public Health Center, examining six dimensions: policy clarity, resource availability, inter-actor communication, implementer characteristics, sociocultural context, and implementer attitudes.

Using a naturalistic qualitative approach between January and June 2025, six purposefully selected informants were involved, including health officers, Puskesmas leadership, local health department officials, and three urban village heads. Data were gathered through direct observation, open interviews, and document analysis, with thematic coding applied concurrently with data collection. Triangulation of sources and methods ensured data validity, and ethical clearance was granted by the Health Polytechnic of the Ministry of Health in Kupang (LB.02.03/1/0036/2025).

Findings revealed that while institutional support exists, barriers such as the absence of community-based Jumantik structures, limited budget allocations, and irregular coordination hinder effective implementation. Additionally, community perceptions delegating responsibility solely to health workers, and insufficient training and monitoring systems, undermine sustainability. In conclusion, although G1R1J has a solid foundation, its success requires systemic improvements across institutional, financial, human resource, training, and communication dimensions.
