BEDA PENGARUH MC.KENZIE EXERCISE DENGAN MAITLAND MOBILIZATION TERHADAP PERBAIKAN NYERI FUNGSIONAL DAN ROM LUMBAL PADA CHRONIC LOW BACK PAIN

Chronic Low Back Pain merupakan kondisi dimana terjadi nyeri punggung yang berlangsung lebih dari 12 minggu dan berulang sehingga menimbulkan berbagai impairment yaitu muscle spasm tightness, facet joint stiffness, dan kelemahan core muscle, yang mengakibatkan nyeri fungsional dan keterbatasan Range of Motion (ROM) lumbal. Tujuan penelitian yaitu untuk membandingkan pengaruh antara Mc.Kenzie Exercise dan Maitland Mobilization terhadap perbaikan nyeri fungsional dan Range of Motion pada penderita chronic low back pain. Jenis penelitian yang digunakan merupakan quasi eksperimen dengan randomized pre test-post test two group desain, yaitu 2 kelompok sampel dimana kelompok perlakuan 1 diberikan Microwave Diathermy, Mc.Kenzie Exercise dan Core Stability Exercise, kelompok perlakuan 2 diberikan Microwave Diathermy, Maitland Mobilization dan Core Stability Exercise. Jumlah sampel 24 orang yang sesuai dengan kriteria inklusi, dirandomisasi dalam 2 kelompok masing-masing sebanyak 12 orang. Data dikumpulkan melalui pengukuran nyeri fungsional menggunakan skala Core Outcome Measurement Index dan pengukuran ROM lumbal menggunakan Bubble Inclinometer. Berdasarkan hasil uji paired sample t pada kelompok perlakuan 1 dihasilkan nilai p=0,000 untuk fleksi, nilai p=0,000 untuk ekstensi, dan p=0,000 untuk COMI-back. Kelompok perlakuan 2 dihasilkan nilai p=0,000 untuk fleksi, nilai p=0,000 untuk ekstensi, dan p=0,000 untuk COMI-back, hal ini menunjukkan terdapat pengaruh yang signifikan pada kelompok perlakuan 1 dan kelompok perlakuan 2 terhadap perbaikan nyeri fungsional serta ROM fleksi-ekstensi lumbal. Kemudian,berdasarkan uji independent sample t diperoleh nilai p=0,000 untuk fleksi, nilai p=0,006 untuk ekstensi, dan nilai p=0,003 untuk COMI-back yang menandakan bahwa ada perbandingan yang signifikan antara kelompok perlakuan 1 dan perlakuan 2 terhadap perbaikan nyeri fungsional dan ROM fleksi-ekstensi lumbal. Pemberian Microwave Diathermy, Core Stability Exercise dan Maitland Mobilization menghasilkan perbaikan nyeri fungsional dan ROM lumbal yang lebih signifikan pada penderita chronic low back pain.

Chronic low back pain is back pain thattlasts more thann12 weeks and recurs,causing various impairments, namely muscle spasm/tightness, facet joint stiffness, and core muscle weakness, which causes functional pain and limitation of lumbar Range of Motion (ROM). The purpose of this study was to compare then effect of Mc.Kenzie Exercise and Maitland Mobilization in patients with chronic low back pain. This type of research his quasi-experimental with a randomized pre test-post test two group design, which uses 2 sample groups where treatment group 1 is given Microwave Diathermy, Mc.Kenzie Exercise and Core Stability Exercise interventions, treatment group 2 is given Microwave Diathermy, Maitland Mobilization and Core Stability Exercise. The sample obtained was 24 people according to the inclusion criteria, then randomized into 2 groups, each group of 12 people. Data collection a obtained through measurement of functional pain using the Core Outcome Measurement Index Scale and measurement of lumbar ROM using a Bubble Inclinometer. Based on the result kof the paired sample t test in treatment group 1, the p value = 0,000 for flexion, p value = 0,000 for extension, and p value = 0,000 for COMI-back. Treatment groups obtained a value of p = 0.000 for flexion, a value of p = 0.000 for extension, and p = 0.000 for COMI-back, this shows that there is a significant effect of treatment group 1 and treatment group 2 on improving functional pain and lumbar flexion-extension ROM. Then, based on the independent sample t test, the p value = 0.000 for flexion, p value = 0.006 for extension, and p value = 0.003 for COMI-back, which means that there is a significant difference between treatment group 1 and treatment group 2 on functional pain improvement and lumbar flexion-extension ROM. The Microwave Diathermy, Core Stability Exercise and Maitland Mobilization produces more significant improvements in functional pain and lumbar ROM inmpatients with chronic low back pain.
