MANAJEMEN FISIOTERAPI OLAHRAGA DAN KEBUGARAN PADA PRE-OP MENISCUS REPAIR DALAM MENANGANI GANGGUAN AKTIVITAS FUNGSIONAL DAN OLAHRAGA LARI MARATHON

Cabang olahraga marathon menuntut pelari menempuh jarak jauh dengan intensitas tinggi, sehingga risiko cedera pada knee joint sangat tinggi. Studi menunjukkan prevalensi lesi meniscus mencapai 72,4% pada pelari marathon amatir. Cedera meniscus berupa horizontal tear dan bucket handle tear dapat menyebabkan gangguan fungsi knee yang signifikan dan memerlukan manajemen fisioterapi komprehensif. Tujuan dari jurnal ini untuk memberikan pengetahuan dan kemampuan dasar tentang teori fisiologi olahraga dan peran fisioterapi pada kasus meniscus tear, serta merancang program intervensi fisioterapi yang tepat. Studi kasus dilakukan pada pasien laki-laki usia 25 tahun, PNS, dengan hobi lari marathon yang mengalami horizontal meniscus tear medial lateral dan bucket handle meniscus tear lateral knee joint sinistra. Asesmen meliputi pemeriksaan fisik, MMT, NRS, MRI, dan tes spesifik. Program fisioterapi pre-operasi dirancang dengan pendekatan exercise therapy, strengthening, dan balancing exercise. Evaluasi menunjukkan penurunan nyeri tekan dari NRS 6 menjadi 4, penurunan tingkat kecemasan dari HARS 25 menjadi 20, namun nilai MMT tetap sama dengan pengurangan nyeri selama aktivitas. Program fisioterapi berhasil mengurangi gejala dan mempersiapkan pasien untuk tindakan operatif selanjutnya. Manajemen fisioterapi pre-operasi pada kasus meniscus tear terbukti efektif dalam mengurangi nyeri, kecemasan, dan mempersiapkan kondisi optimal untuk prosedur pembedahan meniscus repair.

Marathon running requires runners to cover long distances at high intensity, resulting in a high risk of knee joint injuries. Studies show that the prevalence of meniscus lesions reaches 72.4% in amateur marathon runners. Meniscus injuries such as horizontal tears and bucket handle tears can cause significant knee dysfunction and require comprehensive physical therapy management. The purpose of this journal is to provide basic knowledge and skills regarding sports physiology theory and the role of physical therapy in meniscus tear cases, as well as to design an appropriate physical therapy intervention program. A case study was conducted on a 25-year-old male patient, a male civil servant (MCS), with a hobby of marathon running, who experienced a medial-lateral horizontal meniscus tear and a lateral bucket handle meniscus tear in the left knee joint. The assessment included physical examination, MMT, NRS, MRI, and specific tests. The pre-operative physical therapy program was designed using an exercise therapy, strengthening, and balancing exercise approach. The evaluation showed a decrease in pressure pain from NRS 6 to 4, a decrease in anxiety levels from HARS 25 to 20, but the MMT score remained the same with a reduction in pain during activities. The physical therapy program successfully reduced symptoms and prepared the patient for the subsequent surgical procedure. Pre-operative physical therapy management in cases of meniscus tears has proven effective in reducing pain, anxiety, and preparing optimal conditions for meniscus repair surgery.
